Why are some questions skipped on the Health Report?

CommonGround contains "Skip Logic" for certain questions.

Most questions in the CommonGround survey are asked each time a person completes a report. However, there are a few exceptions.

How I Am Doing (HIAD) Section

  • #6: Since my last appointment I used alcohol or street drugs:  This question is asked at every report if the person indicates they have used alcohol or street drugs between 0 and 24 or more days, and if they select “I prefer not to answer at this time”. Alternately, this question is asked once every 6 months if the person indicates “I don’t use alcohol or street drugs”.

Wellness Section

  • Are you currently a smoker? This question is asked at every report if the person indicates they have quit smoking less than 10 years ago.  This question is asked once per year if the person indicates they have quit more than 10 years ago, or that they have never smoked.
  • When was your last checkup with your physical health doctor?  This question is asked if the person indicates their last check-up was more than one year ago, if they aren’t sure of their last check-up, or if they have never had a check-up.  If a person enters a date into the survey, this question won’t be asked unless the date is more than 1 year ago.
  • I am concerned – or my friends and family are concerned – about my use of alcohol, painkillers, or street drugs:  This question is tied to HIAD #6, and is asked or skipped based on the responses to that question. If, on HIAD #6, a person indicates they have used alcohol or street drugs between 0 and 24 or more days, or if they select “I prefer not to answer at this time, this question is asked at every report. The question is asked once every 6 months if the person indicates, on HIAD #6 “I don’t use alcohol or street drugs”.

Medication Section

  • Do you have trouble swallowing pills?  This question is asked at first report, and then every 12 months

Medication Concerns Section

  • This section is non-optional at the first report, and then non-optional every six months. Between the first report, and every subsequent 6 month period, the individual has the option of skipping this section by selecting “no” – indicating they do not have current concerns about their medication.
