How do I review a CommonGround Health Report?

The CommonGround Health Report is one of the most important features of CommonGround, and often the focus of the work clients and staff do within CommonGround. Prior to their appointment with their psychiatric care provider, a client will answer a series of questions designed to help them prepare to participate in shared decision making with their medical staff. 

Client Report Tab

After a CommonGround Health report is generated, staff can open the report and review the work a client has completed prior to the appointment. The Client Report tab condenses all of the client’s responses to the CommonGround survey. Each section or box on the report contains information and should be reviewed.  Items in BLUE may indicate areas of concern, as identified by the client. In addition to the coloring system, a scoring system is used to consolidate the survey responses. 

My Goals for Today's Meeting

When reviewing a Health Report, we recommend beginning with the Goals section. Review any goals they might have included - this is what the client is looking to get out of their meeting with their psychiatric care provider.

Recovery
Next, check out the recovery section. It includes their Power Statement (how the client wants treatment to help), and displays answers on how they have been using their Personal Medicine. If the person and their psychiatric care provider completed a shared decision together at the last meeting, it will also show up here. 

How I Am Doing (HIAD)
The next section of the Health Report is the How I am Doing scale. The HIAD contains the responses to the first 17 questions in the survey. The report displays the responses from the very first time someone completed a health report, their responses from their last appointment, and their current responses. This gives you an opportunity to quickly scan for any changes. Some of these items are highlighted in blue. These are the areas the client indicated they might be experiencing a challenge in and are a potential area for discussion.

You don’t have to know what each of the numbers on the Health Report means - the report will assist you. Anything that has a little dotted line under is a link to more information. So, clicking on a 4, for example, you can see the question and how this person responded. 

Additionally, clicking on the question identifier will show you the full question and answer set. Clicking here also brings up resources from the library which might be helpful in this area. 

My Meds
The Medicine that’s entered in the client’s profile is then automatically pulled into their Health Report. They are asked about how they have been using their medicine since their last appointment with you. All of this is then displayed on the completed report. Take note - this is not a prescribing list. There are no dosages or frequencies. It is designed to be a medication education tool.
Each medication contains a link to brief information about the medication and a link to a consumer-friendly fact sheet - in both English and Spanish. 

My Concerns about Mental Health Medications

The next section on the health report is the "My Concerns about Mental Health Medications" section. These are the 11 questions people answer about common medication concerns. Clients must answer these questions on their first report, and at least every 6 months. In between, they can skip this section if they don’t have concerns.

Just like the HIAD section, concern areas show up in blue, and there are links to display the question, their responses, and resources which might help.

If someone has endorsed a particular concern, it also shows up under the “this visit” column as a blue triangle. 

Additionally, there is a display of responses from the previous visits. Past concerns show in this table as a black dot. 

As people complete more health reports, you can use this area to scan for any ongoing concerns which may be a barrier for using psychiatric medicine. As you know, there are many reasons why a person might stop taking their medicine. Identifying concerns clients might have about their medicine can help the team support the client through any decisional uncertainty about their medicine - supporting their journey toward optimal use of medication.

My Wellness

The final section of the health report contains information related to physical health. The My Wellness section contains concern areas highlighted in blue and resources to support folks in these areas. 

As you can see, the CommonGround Health report is designed to support you in the important work you are doing. Incorporating this into your existing workflow can help ensure individual client preferences, needs, and values guide your shared-decision making process.
